Fax
Complete registration form in its entirety, including payment information, and fax
to the ASHI Registration Department at 877-716-6404.
Mail
Complete registration form including credit card information or a check made

Checks must be made payable to ASHI in U.S. dollars and drawn on a U.S. bank.
ASHI Cancellation/Refund Policy:
All cancellation requests must be made in writing, to Brooke Bilofsky at bbilofsky@ahint.com or mailed to the ASHI Office. A $75 processing fee will be charged for all cancellations postmarked on or before August 26, 2010. Requests for refunds on or after August 27, 2010 will not be honored. All refunds will be issued after the Conference and registrations are non-transferable.
